fix(eve): settle Discord approval messages

Summary

Related to #1021. Discord tool-approval buttons now preserve the submitting user’s identity and leave the shared message unchanged until approval.settled. Settlement then clears the controls and shows whether the approval was approved or cancelled, with the responder.

This is a sibling stacked PR with #2251, both based on #2209.

Validation

Focused Discord interaction and lifecycle coverage verifies responder auth, durable response state, message settlement edits, and mention suppression. Typecheck and invariant checks passed locally.

On the Discord HITL freeform path (prefix eve_input_freeform:, 19 chars), the current base64url-JSON-array custom_id encoding leaves almost no headroom under Discord's 100-char limit — a freeform question whose provider tool-call id is ≥30 characters plus a 19-digit conversation id produces a custom_id of ≥101 chars, making encodeHitlCustomId throw and breaking the input.requested handler so the prompt is never posted.
