Port the PR/MR review-hand-off gate from dotfiles - #64

dotfiles gained a "Keeping PRs/MRs in sync" gate (refresh the title and description before every review hand-off, including after rework) and a `gh pr edit` reference. claude-shared only had an older, shorter MR section. Bring both skills level with dotfiles so claude-shared is the complete source before dotfiles stops carrying copies. Co-Authored-By: Claude Opus 5.5 (1M context) <noreply@anthropic.com>
