Managed checkouts now keep the repository directory name, with the worktree/branch name as an intermediate path segment. create makes the full directory first, then adds the worktree; migrate moves main (via rename + repair) and old-layout features into the nested paths.
Remove non-main managed worktrees (soft-delete merged branches, keep unmerged ones), then collapse <root>/main/<repo> to <root>. Refuse dirty worktrees unless --force. Shell wrapper cds to the collapsed root.
