retro: Prevent baseline checks from substituting for requested implementation evidence #164
Description
Objective
Promote candidate #125 into one minimal, human-reviewed retro PR that prevents existing green checks from being accepted as evidence of a newly requested implementation delta, then retire the candidate after merge.
Context & references
- Epic: Epic: Reduce development friction without weakening verification #119.
- Derived from: Retro hygiene review 2026-09 #118 and retro-candidate: Passing baseline tests mistaken for new-feature completion #125.
- Authorization: owner decisions D1-D3 in Retro hygiene review 2026-09 #118 (comment). Asset selection belongs inside this Task, using the retro classification table; the hygiene snapshot is not an implementation plan.
- Startup: this source template is intentionally untuned. Run
bash .github/scripts/tuning-status.sh --quiet; exit 1 is covered for approved Epic: Reduce development friction without weakening verification #119 work and its supervisor/worker/resumed sessions by Epic: Reduce development friction without weakening verification #119 (comment). Acknowledge untuned, carry that link, keep CUSTOMIZE, and do not repeat onboarding. Other exit failures are errors, not a decline. - Read

.github/docs/agreements/adr/ADR-0004-hotl-governance-sensors.mdgoverns the sensor/runtime boundary. No new REQ or ADR is requested. - Evidence 1: Support single-maintainer governance with mandatory CI and no required peer approval #124 (comment). Pre-existing solo/team suites passed, but the requested feature, focused regressions, new commits, and PR did not exist.
- Evidence 2: Support single-maintainer governance with mandatory CI and no required peer approval #124 (comment). Word counts or absent comparisons were substituted for production-state comparisons. Preserve the separately corroborated real reds, hashes, and test-before-fix ordering; do not relabel them fabricated.
- Evidence 3 (latest): Support single-maintainer governance with mandatory CI and no required peer approval #124 (comment). The worker received a new G3/G5 correction but ran existing green verification against unchanged head 791 and reported completion without the new source/test delta or capture. No runtime cause was demonstrated. The supervisor rejected completion; the focused continuation required a real delta, ordered test/fix commits and red-then-fixed evidence, not merely a changed SHA.
- Candidate ledger: retro-candidate: Passing baseline tests mistaken for new-feature completion #125 filing plus its two
Occurrence:comments retro-candidate: Passing baseline tests mistaken for new-feature completion #125 (comment) and retro-candidate: Passing baseline tests mistaken for new-feature completion #125 (comment) total three occurrences, above the threshold of two. Read all three original records and the candidate in full. - Budget at the coordinator's 2026-09-20 render: AGENTS.md 117 lines; .github/copilot-instructions.md 125 lines. Recount at execution time. The session-orchestration skill is already near its 500-line ceiling; prefer existing on-demand references over growth.
Acceptance criteria
- AC1: The Plan and PR classify the observed root cause using the retro skill table, cite all three occurrence links, and explain why the chosen owned asset is the most deterministic feasible host. Prefer an executable check/test over advisory prose where it can actually measure the claim; do not pretend a SHA change, test count, text match, task_complete signal, or generic suite pass measures the requested behavior.
- AC2: Deliver the smallest asset diff that would have prevented acceptance of the latest occurrence. It must distinguish baseline verification from the latest requested source/test change, bind evidence to that request and actual delta, and require the applicable new regression, ordered test/fix commits and captured red-then-fixed evidence before implementation completion. Do not invent historical red results or turn this into an unconditional requirement for artificial source/test edits on documentation-only work. Preserve credited evidence and the rule that upper tiers inspect evidence rather than rebuild it.
- AC3: Before the asset change, define and record the focused negative and positive cases. Reject (a) unchanged baseline-green evidence for a new correction, (b) a changed SHA without the requested behavior/delta, and (c) word counts substituted for state comparisons. Accept real request-specific delta and ordered red/fixed evidence; keep legitimate documentation-only verification possible. If changing executable logic, add focused regressions first and record their genuine red before the fix and green after it, then run affected existing suites. If the classified fix is procedural only, provide a before/after scenario walkthrough tied to exact text and explain the deterministic limit; do not claim runtime enforcement or observed future compliance.
- AC4: Use only the necessary subset of the candidate assets in File ownership. Keep the Task form and body template synchronized if either changes. For every line added to AGENTS.md or .github/copilot-instructions.md remove or demote a line in that file, report before/after counts and the trade, and preserve all surface ceilings. No unrelated budget trimming is requested.
- AC5: Append exactly one row to
.github/docs/agreements/retro-log.md, preserving every existing row. Use the retro skill's four-cell format| YYYY-MM-DD | Failure class | Fix (asset + change) | Evidence |; use the actual date and selected asset, and cite both retro-candidate: Passing baseline tests mistaken for new-feature completion #125 and Retro hygiene review 2026-09 #118 in Evidence. Obtain at least one human approval for this agreements-tree change per docs.instructions.md and the retro Procedure before merge. A custom reviewer audit is not human approval. - AC6: Publish exactly one PR titled
retro: <what it prevents>with the full PR template, Plan link, all acceptance evidence, explicit deviations and required custom-reviewer audit. Because AC7 is post-merge, its primary Task link usesRefsfollowed by this Task's issue reference; do not use an automatic closing reference for this Task or retro-candidate: Passing baseline tests mistaken for new-feature completion #125 and do not batch the candidate as a second Task. - AC7 (post-merge, supervisor/coordinator): Verify the PR is merged, append a fix-link comment to retro-candidate: Passing baseline tests mistaken for new-feature completion #125 explaining that the landed retro PR retires this failure class, and manually mark the candidate completed. Then record this Task's post-merge evidence and Outcome before manually marking this Task completed. Until these actions are verified, AC7 stays pending and no completed Task outcome is valid. Filing this Task or opening its PR does not retire retro-candidate: Passing baseline tests mistaken for new-feature completion #125.
Out of scope
- Reimplementing Support single-maintainer governance with mandatory CI and no required peer approval #124, reopening its historical exceptions, reconstructing lost evidence, or changing its closed ledger.
- Platform-baseline changes, other retro candidates, a new runtime completion hook, authenticated identity assertions, telemetry, or additional approval stages beyond existing risk and human-merge gates.
- Workflow files, workflow permissions/triggers, live rulesets/variables, required-check names, auto-merge, or bypasses. Never weaken existing checks. If the selected fix needs an unowned path or workflow change, stop and request replan before editing it.
- Substantive REQ/ADR changes, unrelated agreement riders, broad refactoring, and implementation of the parallel platform-review Task.

Verification
Run these mandatory repository checks and record actual results; all must exit 0:
bash .github/scripts/check-copilot-surface.sh
bash .github/scripts/check-template-sync.sh
bash .github/scripts/tests/run-tests.sh ritual escalation-wording ownership-overlap retro-hygiene tuning-status
git diff --check
wc -l AGENTS.md .github/copilot-instructions.md
git status --shortThe selected suites cover the allowed ritual guard/library and their existing chronology/linkage/dispatch/adoption/record tests, ownership grammar, escalation wording, hygiene and tuning behavior. Run any additional existing suite that consumes a touched asset; for guard/library changes also run bash .github/scripts/tests/run-tests.sh retarget-freshness ci-event-isolation. Keep Bash 3.2 compatibility for changed shell logic. Record focused AC3 commands/results and exact-head diff/commit evidence in the PR; existing green suites alone do not establish AC2 or AC3.
Inspect the PR diff and git log --reverse --format='%H %cI %s' <base>..HEAD against the approved Plan, selected paths, test/fix order, one-row log addition and budget trade. After publication, substitute the actual numbers into gh pr view <pr> --json body, files, headRefOid,reviews, state, mergedAt, gh pr checks <pr>, and gh issue view <this-task> --comments. For AC7, read gh issue view 125 --json state, comments after the manual disposition and link its fix comment in the Task outcome. Do not manufacture evidence or mark pending human/post-merge criteria passed.
Routing
- Surface: exec:app
- Suggested role: default
- Model/reasoning tier: high-reasoning; repository implementation setting is
auto. Respect the explicit Epic Epic: Reduce development friction without weakening verification #119 owner preference for new sessions (gpt-6-astra) when dispatching, with explicit model and mode arguments; do not silently substitute a default. - Parallel-safe: yes with the platform-baseline review; this Task owns the retro log and bounded completion-evidence assets, never platform-capability-baseline.tsv. No blocked-by edge is needed for these disjoint sets; recheck other active ownership before dispatch.
- Risk/review: risk:high applies because asset selection may change a completion operating contract, the constitution or a CI guard, not merely editorial wording. The supervisor posts the Plan and waits for the existing explicit approval before worker implementation. Exactly one custom
reviewersession is mandatory for this governance-surface Task, regardless of final asset choice.
Handoff notes
Use one default-agent supervisor, one default-agent worker in autopilot, and one custom reviewer session; no supervisor-direct exemption. The supervisor owns Task ritual comments and post-merge disposition; the worker owns the PR and evidence. Create and verify the worker before publishing its dispatch, then release it under the approved Plan. Use supported, exposed Rubber Duck in-loop; if unavailable, record that once in the existing Plan or PR evidence and continue the other review duties without inventing a replacement gate. Decomposition and ai:ready certify a complete brief, not dispatch or merge permission. The coordinator owns #118 disposition; do not change that issue. Preserve the English, plain-ASCII durable-artifact convention and avoid closing-keyword/issue-number phrases in issue comments.

Starting in session Task #164 supervisor, branch task/164-retro-completion-evidence
Task: #164
mochan-tk commentedon Sep 20, 2026
Plan
Task: #164
Goal and acceptance: deliver one minimal retro for #125 under #164, not a repair of #124. AC1 classifies all three occurrences and justifies the host; AC2 binds completion evidence to the latest requested delta; AC3 exercises the negative/positive scenarios below; AC4 limits paths and preserves budgets; AC5 appends one retro-log row with human approval before merge; AC6 publishes one fully evidenced draft PR and one independent audit; AC7 remains pending until coordinator-authorized post-merge disposition.
Classification and selected assets:
Implementation procedure after explicit approval:
AC3 cases defined BEFORE asset changes (procedural before/after walkthrough, not runtime tests):
Ownership, budgets and exclusions:
Verification assigned to the worker (capture actual results; baseline checks do not establish AC2/AC3):
Authority and handoff:
STOP: Task #164 supervisor will create no worker and perform no file edit, commit, branch push or PR creation until the coordinator posts a comment starting '## Approval (risk:high Plan gate)' on #164 explicitly approving this Plan.

Approval (risk:high Plan gate) - coordinator, Epic #119
Approved Plan of record: #164 (comment) (posted 2026-09-20T00:22:10Z, unedited at approval time: REST created_at = updated_at), following claim 5746366221 (00:20:28Z) by the supervisor session "Task #164 supervisor" (project session and tool-visible ID 7d8f4746-5a3d-455a-adec-80dc44337b64), actual branch

The supervisor stopped after the Plan as the Routing block requires. Coordinator re-read on 2026-09-28 at approx. 17:45Z, immediately before this comment: the Task body has never been edited (GraphQL userContentEdits totalCount 0; raw body md5f7789320f6e15630b8617c50a1fee2c0, the value recorded before dispatch); this issue still holds exactly the claim and the Plan;origin/mainhas not moved since the Plan; no remote branch matches this Task; the only open PRs are #166 (.github/scripts/platform-capability-baseline.tsvonly) and #104 (governance-status.shand its test); the supervisor worktree is clean with noplan.md;ownership-overlap.shfor 164 and 163 returnsNO_OVERLAP; line counts onorigin/mainare AGENTS.md 117,.github/copilot-instructions.md125, verification skill 137 and session-orchestration skill 500. No worker exists and no file has changed.Authority. The owner, asked in the Epic #119 coordinator session after the coordinator compared the Plan against the work order (Objective, Context, AC1-AC7, Out of scope, File ownership, Verification, Routing, Handoff notes) and recorded that comparison in Epic #119 comment 5746403075, approved this Plan with the one condition below. The first request (2026-09-20 approx. 00:27Z) returned a tool error without an answer; the owner's explicit answer, selecting approval with the stated condition, arrived on 2026-09-29 JST (2026-09-28 approx. 17:42Z), and the coordinator re-verified the facts above before posting. Dispatch decision and terms: Epic #119 comment 5746350040; creation record 5746372458; M1 verification 5746403075; planner rationale 5746293307; #118 review record 5746263031; onboarding deferral in scope: 5668795167 (startup exit 1 acknowledged as untuned, nothing more; the deferral is not risk approval). This comment is the single risk-gate approval for #164. It does not approve the PR, ready-for-review conversion or merge, and it is not the AC5 human approval of the agreements change: both are a separate owner decision on the PR after record verification and the reviewer audit, recorded by the coordinator on the PR (precedent: PR #158 comment 5742874119). It presents no earlier approval as a new one.
Condition. The PR title must mirror the Task title exactly:
retro: Prevent baseline checks from substituting for requested implementation evidence. The draft title in Plan step 5 omits the wordrequested; this condition corrects that string only and changes no scope, ownership or acceptance criterion.Permitted scope. The Plan's two-path subset of the Task's File ownership, and nothing else:
.github/skills/verification/SKILL.md(a compact request-specific completion procedure placed in the existing Test-first work orders / Pre-PR checklist flow, not duplicated in other assets) and.github/docs/agreements/retro-log.md(exactly one appended four-cell row| YYYY-MM-DD | Failure class | Fix (asset + change) | Evidence |with the actual execution date, the selected asset, and both #125 and #118 in Evidence; every existing row preserved byte-for-byte). The procedure binds implementation-completion evidence to the latest request/Plan/correction link, the pre-correction base/head and the actual relevant source/test delta; for executable behavior changes it requires the applicable new focused regression, a genuine captured failure before the production fix, ordered test/fix commits and captured fixed success at the identified revision; claimed state measurements compare named states or paths directly; missing historical evidence stays missing; documentation-only work proves its requested document delta without artificial source/test edits or invented red runs; separately credited earlier evidence and upper-tier inspection (not rebuilding) are preserved. The other eleven owned paths (session-orchestration skill and its ritual reference, PR template, Task form and body template, ritual guard, library and tests, AGENTS.md,.github/copilot-instructions.md) stay byte-for-byte unchanged; AGENTS.md and.github/copilot-instructions.mdkeep 117 and 125 lines; the verification skill's before/after counts are recorded and every surface ceiling holds. No executable logic change, no new REQ or ADR, no agreements wording rider, no new dependency or service.Execution authority. One worker session (default agent, explicit
gpt-6-astra, explicitautopilot,coordinate_with_creator: true,notify_on_idle: always, nobase_branch) created by the supervisor in the order create -> confirm actual identity/branch/worktree withget_session->task-ritual.shrender/inspect/preflight ->Dispatching workerrecord on this issue -> explicit release, using the complete Worker kickoff template with the Task's verbatim File ownership and Verification plus the narrower approved two-path selection; the worker's startup hold may include renaming its branch totask-164-retro-completion-evidence(managed prefix accepted) before any edit, and its first commit must postdate the dispatch record. Supervisor-direct implementation is not eligible for this Task. The worker executes Plan steps 1-5: it records the pre-change inputs and the exact existing text/line references of each AC3 case (N1 unchanged baseline, N2 unrelated new SHA, N3 surrogate comparison, P1 real correction, P2 documentation only) before editing, then quotes the exact before/after text and line references for every case decision in the PR as a procedural walkthrough with the deterministic limit stated; it runs every Verification command of the Task (check-copilot-surface.sh,check-template-sync.sh,run-tests.sh ritual escalation-wording ownership-overlap retro-hygiene tuning-status,git diff --check,wc -l,git status --short, the base..HEAD diff and commit log) and records its actual base SHA and outputs; commits with the Copilot App trailer; and opens the PR as a DRAFT through the app tool with the exact title above, the full PR template, the primary linkRefsto this Task, no closing keyword for this Task, #125 or any other issue anywhere (PR text, commits, evidence, comments), links to the Plan and this Approval, one evidence row per AC1-AC7 with AC5 human approval and AC7 honestly pending, and explicit Deviations. The worker never posts ritual comments on this issue. Within this scope, edit -> verify -> record needs no further approval. Re-approval is required for any material change to ownership (including touching any unselected owned path or any always-on file), scope, authority or acceptance criteria, including any move of the rule into guard or library behavior: post a revised## Plan(never edit this comment or the original Plan) and stop again. If the Plan cannot be satisfied as specified, escalate per AGENTS.md section 6 with a comment on this issue stating the situation and the options, then stop and report; the coordinator applies the matchingneeds:replanorneeds:humanlabel (label changes stay with the coordinator per decision 5746350040).Risk.
risk:highbecause the change amends the verification procedure every agent follows and appends to the agreements tree. Accepted on these conditions: the diff is procedural only and claims neither runtime enforcement nor observed future compliance; no test, check or lint is deleted, skipped or weakened, and no guard, library, test, workflow, ruleset, variable or required check changes; the always-on files are untouched; documentation-only verification stays possible; after a## Review checkpointon this issue, the mandatory independent customrevieweraudit (exactly one session, agentreviewer, explicitgpt-6-astra, explicitautopilot,coordinate_with_creator: false, nobase_branch) is published as a non-approving comment on the PR while it is still a draft, checking Task linkage, every AC evidence row, verification integrity, the two-path ownership, the Budget rule, deviations and governance safety; Rubber Duck is claimed only where actually run (the supervisor stated its absence once in the Plan);## Outcomeis posted before any session message and is expected to beblockedon the AC5 human approval, human merge authority and the AC7 post-merge steps, nevercompletedbefore AC7 is verified; human merge authority is unchanged.Post-merge (AC7). Task: #164
Target: one forthcoming DRAFT PR titled exactly 'retro: Prevent baseline checks from substituting for requested implementation evidence', primary Task link 'Refs #164'.
Scope: ONLY .github/skills/verification/SKILL.md and .github/docs/agreements/retro-log.md, procedural change plus exactly one appended four-cell row; all other paths unchanged.
Plan: #164 (comment)
Approval: #164 (comment)
Created this default-agent worker with explicit gpt-6-astra/autopilot, coordinate_with_creator true, notify_on_idle always, no base_branch. get_session confirmed the actual tool-visible ID above, renamed branch and isolated worktree liner-takashi-kawamoto-reimagined-memory before this record; the worker separately reported app workspace ID e6a7128f-0bd7-4f1a-9ebf-833121d3eca3. Its startup hold reports clean HEAD/base 446071c and no file edits/commits/push/PR. Supervisor refreshed origin/main at that same SHA, ledger has the unedited claim/Plan/Approval, budgets remain 117/125/137/500 and ownership-overlap 164 163 is NO_OVERLAP. Full kickoff includes verbatim ownership/Verification, the narrower approved scope, five pre-change AC3 cases, conditioned title, evidence/human/post-merge limits. An explicit release message will follow publication/readback; first edit and commit must follow this dispatch.
Current-session tool observation corrects the earlier Plan availability statement: the resumed supervisor exposes a Rubber Duck task tool and has now run one read-only in-loop Plan critique. It is advisory, not approval or the independent Task audit. Useful cautions: tie relevant delta to the particular correction, separate implementer capture from upper-tier inspection, use direct state comparisons, keep documentation-only exemption adjacent and scope mixed work by executable portions. Its suggested universal rule that any check already passing on the base disqualifies evidence is NOT adopted: baseline checks remain credited as baseline; request-specific assertions and actual delta, not an unconditional rerun or new scope, determine completion. No approved scope or acceptance change follows.
No ready/merge/approve/label/governance action; AC5 human approval and AC7 remain pending. No action on candidate #125 before the coordinator's explicit post-merge instruction. Task #164 supervisor retains ritual, checkpoint and Outcome duties; worker owns its PR evidence and reports only to the supervisor.

Owner-account confirmation of records 5875471874 and 5875535432 - coordinator, Epic #119
Posted from the owner account
mochan-tk(author_association OWNER) at the owner's direction, given in the Epic #119 coordinator session on 2026-09-28 (UTC) after the coordinator reported the identity change below. This comment adds attribution only: it approves nothing new, changes no scope, and edits no existing comment. Two ledger comments on this issue were published under the Copilot app's linked account
liner-takashi-kawamoto(author_association NONE, pull-only on this repository). The session environments now receive that account's token as GH_TOKEN (observed on 2026-09-28; every earlier comment on this issue, including the claim and the Plan, was made asmochan-tk, which is still logged in through the keyring). Neither writer noticed before posting:## Approval (risk:high Plan gate)bc37a138-f374-4c18-a078-f0e9d7a19de7Dispatching worker7d8f4746-5a3d-455a-adec-80dc44337b64152eb473-d330-4964-a56d-b5c25064e0f7(get_session: name "Task #164 worker", created 17:50:42Z, branchliner-takashi-kawamoto-task-164-retro-completion-evidence, worktreeliner-takashi-kawamoto-reimagined-memory, zero diff), the exact conditioned PR title, the two approved paths, and links to the Plan and the Approval; unedited (created_at = updated_at)The owner confirms both as genuine records of this Task's ledger, with the same force as if
mochan-tkhad posted them; 5875471874 remains the single risk-gate approval for this Task. Theliner-takashi-kawamoto-prefix in the worker's branch is the app's managed prefix and is accepted as the managed-prefix equivalent under AGENTS.md section 4.The owner also decided that from now on every
ghandgitoperation of the coordinator, the supervisor and the worker runs asmochan-tkwith GH_TOKEN unset (keyring credentials), each session first verifying thatenv -u GH_TOKEN gh api user --jq .loginreturnsmochan-tk, and that app tools which would write to GitHub under the linked account are not used for this Task. The PR is therefore pushed and opened withenv -u GH_TOKEN git pushandenv -u GH_TOKEN gh pr create --draftundermochan-tk; the PR records this as a deviation from the app-tool default.State at this confirmation: the worker's release had been delivered, then the coordinator put the supervisor and the worker on hold at approx. 17:53Z. The worker reports HEAD
446071c76f14f5fbda37a0eef1b6eafa0a3ab897, an emptygit status --short, and no commit, push, PR or GitHub write. No remote branch or PR exists for this Task. The coordinator resumes both sessions after this comment is read back.mochan-tk commentedon Sep 28, 2026
Publication route blocker - Task #164 supervisor
Owner-account confirmation #164 (comment) was read in full and verified as mochan-tk / OWNER, unedited. The supervisor verified the requested token-unset identity as mochan-tk and relayed the identity rule verbatim to the existing worker. Existing dispatch and branch remain unchanged; no replacement record is needed.
The approved local two-file implementation and verification can resume. PR creation alone is blocked by the currently available publication routes: the designated app PR tool would use the linked account prohibited by the owner, while CLI PR creation is not an available fallback in this session without explicit fallback authorization returned by that tool. The supervisor will neither probe a prohibited write nor bypass this limitation. The worker has been instructed to prepare the exact-title draft PR body and exact-head evidence without creating a PR by either route, and to report its preserved branch/head.
Options for coordinator/owner disposition: repair the linked-account binding to the approved owner identity and explicitly authorize the app publication route, or have the owner publish the prepared draft PR under mochan-tk and provide its URL for worker evidence and the existing reviewer sequence. No new implementation scope or approval stage is proposed. Request coordinator disposition of this publication-only blocker; label decisions remain with the coordinator. No ready, merge, approve, label, governance or candidate action is taken here.

Publication transport blocker - Task #164 supervisor
The existing worker reports a second publication blocker after completing its authorized local two-path change: immediately after the token-unset gh identity check returned mochan-tk, env -u GH_TOKEN git push -u origin liner-takashi-kawamoto-task-164-retro-completion-evidence failed with exit 128 / HTTP 403, permission denied to liner-takashi-kawamoto. This proves the gh API identity check did not establish the identity used by that Git transport; no root cause beyond the observed mismatch is asserted.
Worker-preserved local head: 461e2fb, on the already dispatched branch/worktree. Worker reports author/committer mochan-tk, exactly two approved paths (+28/-2), required selected suites 10 files / 221 cases passing, surface/template/escalation/diff checks passing, counts 117/125/162/500, and a pre-edit five-case record. These are worker-reported results awaiting supervisor evidence inspection, not a Task-completion claim. The prepared PR body artifact will carry the full evidence. No PR exists or was attempted.
The worker performed no retry, authentication workaround or configuration change. Publication remains blocked by the recorded PR-tool route constraint and this Git transport mismatch. Request coordinator/owner resolution of owner-authenticated transport and PR publication; no implementation scope change, new worker, replacement dispatch, label change or candidate action is proposed. Preserve the local commit and stop publication attempts until explicit instruction.

The authorized local implementation is preserved, not published or accepted as a completed Task. Supervisor inspection of the actual Git object confirms base 446071c -> local head 461e2fb; commit timestamp 2026-09-29T03:02:05+09:00 follows dispatch, author/committer mochan-tk and Copilot App trailer present. Exactly two paths changed: verification/SKILL.md +27/-2 and retro-log.md +1/-0. The latter appends the 2026-09-29 JST row citing candidate #125 and owner review #118. No other implementation path changed. get_session confirms the same worker/branch; the worker reports a clean final checkout and no push retry. The supervisor read the exact diff, prepared full PR body, pre-change five-case record and final suite record, rather than rebuilding the artifact. A fresh PR query for the dispatched branch returned an empty list.
Evidence:
Local verification record inspected: check-copilot-surface, check-template-sync, selected ritual/escalation-wording/ownership-overlap/retro-hygiene/tuning-status suites, diff-check, counts/status, and direct escalation-wording consumer check all reported exit 0. The final retained suite log has 10 files / 221 cases / zero failures. These compatibility checks do not substitute for the request-specific procedural walkthrough. This documentation-only Task has no invented red run or test/fix commit sequence. The worker ran exposed Rubber Duck in-loop and recorded its dispositions; it is not an independent Task audit or human approval.
Blockers: publication-route record #164 (comment) and Git-transport record #164 (comment). A verified token-unset gh identity of mochan-tk did not establish Git transport identity: the push returned exit 128 / HTTP 403, permission denied to liner-takashi-kawamoto. No authentication workaround/configuration change or retry occurred. The designated PR-tool route remains incompatible with the owner prohibition on linked-account writes; neither route was attempted.
Preservation: worker tool-visible ID 152eb473-d330-4964-a56d-b5c25064e0f7, app workspace ID e6a7128f-0bd7-4f1a-9ebf-833121d3eca3, branch liner-takashi-kawamoto-task-164-retro-completion-evidence, worktree liner-takashi-kawamoto-reimagined-memory, local head above. Prepared body is task-164-pr-body.md in that worker session artifacts, SHA256 4460e77f1e741f5e37f24e6a4ced4e6f3a47458ad08cef9ee5f376ef3b9932eb; supporting records task-164-before.txt and task-164-tests-final.log. Local artifacts are recovery material, not a substitute for publishing the PR evidence. No archive/discard authorized; coordinator owns disposition.
Deviations: owner-account handling and unresolved publication routes prevented the approved draft-PR/audit handoff. Implementation stayed within the approved two paths; no executable change, new approval stage or historical reconstruction. AC5 approval, AC6 publication/audit and AC7 are explicitly not satisfied.
Follow-ups: coordinator/owner resolves authenticated Git transport and allowed PR publication, then explicitly resumes the existing worker for exactly one draft PR with the conditioned title. After real PR/CI readback, supervisor records Review checkpoint, creates exactly one custom reviewer and reads its non-approving PR audit, then posts a fresh Outcome. Human approval/merge authority and explicit post-merge instruction remain with coordinator. No ready/merge/approve/label/governance/candidate action and no other Task action performed.

Coordinator disposition: publication route and Git transport (owner decision) - coordinator, Epic #119
Resolves the publication-only blockers 5875606874 and 5875697161 recorded by Task #164 supervisor. The Outcome 5875732461 (
blocked) stays as the accurate record of that moment; the supervisor posts a fresh Outcome after the audit, as it planned. Scope, ownership, acceptance criteria and Approval 5875471874 are unchanged; this adds no approval stage and replaces no record. Written asmochan-tk(OWNER) with GH_TOKEN unset.Owner decision (publication route). Asked in the Epic #119 coordinator session on 2026-09-28 (answered between 18:01Z and 18:14Z UTC), the owner chose: the worker pushes its branch as
mochan-tkand the coordinator publishes the prepared body unchanged as the DRAFT PR withenv -u GH_TOKEN gh pr create --draft, then hands the URL to the supervisor. This changes only who runsgh pr createin the Approval's execution authority and in confirmation 5875589353; the worker keeps its branch, its commit and all PR-evidence duties, and the supervisor keeps review, checkpoint and Outcome duties. The app PR tool is not used: it writes under the linked account, and it covers only the calling session's own branch.Transport (coordinator resolution within the owner's identity decision). The HTTPS push fails because the app-configured Git credential helper supplies the linked account's token even when GH_TOKEN is unset (record 5875697161: exit 128, HTTP 403, permission denied to
liner-takashi-kawamoto). The owner's SSH key authenticates as the owner account: on 2026-09-28 at approx. 18:31Z the coordinator observedssh -T git@github.comprintHi mochan-tk! The worker therefore pushes over SSH with an explicit URL and refspec, changing no remote, credential or Git configuration and setting no upstream:Sequence.

No ready, merge, approve, label, ruleset, variable or workflow action; no action on #125 before the coordinator's post-merge instruction; AC5 human approval of the retro-log row remains a separate owner decision on the PR. Evidence publication handoff - Task #164 supervisor
PR #169 now exists as an OPEN draft by mochan-tk, with the exact approved title and head 461e2fb. Supervisor independently read all five checks as passing (CI run 36466919136, ritual run 36466919206), the two-path +28/-2 diff, actual base a897a67 and merge-base 446071c. The only intervening main-path change is platform-capability-baseline.tsv; no branch update or scope change is performed.
The existing worker is preparing the post-publication evidence body, but reports that its available PR-edit tool route requires the designated updater and does not permit the requested direct REST update. The owner prohibits the linked-account updater, so the worker has attempted neither route. Request coordinator publication of the prepared evidence body under the owner account, analogous to the creation handoff in disposition 5876152046, followed by exact body/head readback. This changes only the publisher, not implementation, evidence ownership or audit requirements. No new worker/reviewer, no authentication workaround, no ready/merge/approve/label/candidate action. Review checkpoint and the single independent reviewer follow the durably updated PR evidence.

Coordinator resolution: publisher of PR #169 body updates - coordinator, Epic #119
Resolves the evidence-publication handoff 5876323939 within the owner's identity decision (confirmation 5875589353) and publication-route decision (disposition 5876152046). Written as
mochan-tk(OWNER) with GH_TOKEN unset. It changes only who transmits the PR #169 body text: the content, the worker's evidence ownership, scope, ownership, acceptance criteria, Approval 5875471874 and the audit duties are unchanged, and it amends only step 5 of the coordinator's own disposition 5876152046, which had assigned body updates to the worker. The worker's session tools route PR body edits through the app's PR updater, which writes under the linked account the owner excluded, so the worker attempted neither that updater nor a direct REST update. The coordinator's app updater has the same linked-account problem, so the owner-account route is a REST call run by the coordinator:
461e2fb2e58fd2fd8a276bc6dcf7fd4f38d3d48d(or at the head the supervisor names if a later commit was made within the Approval), that the text is ASCII, keepsRefs #164and contains no closing-keyword phrase; then runsenv -u GH_TOKEN gh api -X PATCH repos/mochan-tk/agentic-dev-kit-for-copilot/pulls/169 -F body=@<file>; reads back body equality and the head; changes nothing else (title, base, draft state, labels, reviewers); and replies to the supervisor with the result. This standing resolution covers every body update of PR #169 until the PR is merged or closed, including the linkage update after the audit and the Outcome. The Epic #119 record lists each published SHA256. The same identity rule applies to the reviewer's audit: it is published as
mochan-tk(for a top-level PR comment, env -u GH_TOKEN gh api -X POST repos/mochan-tk/agentic-dev-kit-for-copilot/issues/169/comments -F body=@<file>), or, if the reviewer's tools cannot do that, through the skill's requester-publication fallback with its provenance stated. No ready, merge, approve, label or candidate action is taken here.mochan-tk commentedon Sep 28, 2026
Review checkpoint
Task #164 supervisor independently verified PR #169 before dispatching the single custom reviewer: OPEN/DRAFT, author mochan-tk, exact conditioned title, head 461e2fb, base main a897a67, merge-base 446071c. Diff is exactly the two approved paths (+28/-2); the intervening main change is disjoint platform baseline data.
The PR body was published by the coordinator under owner-account standing resolution 5876376936, without changing worker-authored content. Supervisor independently compared REST-parsed body bytes against the frozen prepared file: equal, SHA256 167fe3d6afd79f6275fdff20f9f49233407682bcdeb72c8d45c5317089a70b39. It includes all AC rows, original occurrence links, exact five-case before/after walkthrough, actual local evidence, base facts and explicit publication/transport deviations. AC5 human approval and AC7 remain pending; AC6 independent audit is next.
All five current checks pass: quality, copilot-surface, scaffold-self-check and windows-launcher from run 36466919136; latest task-ritual from run 36468097809. The latest ritual run is pull_request, completed/success, exact head 461e2fb. No suite was rebuilt by the supervisor. Earlier blocked Outcome 5875732461 remains unchanged historical evidence.
Next: create exactly one custom reviewer session, agent reviewer, explicit gpt-6-astra/autopilot, coordination false, no base_branch, to audit this Task/Plan/Approval/two-path claim and publish one non-approving PR audit. Reviewer must use GH_TOKEN-unset gh/git and verify mochan-tk before any write; if top-level comment publication is unavailable, return its audit marked unrecorded for requester publication with provenance. The existing Refs linkage is required for this post-merge Task; no automatic closing link or formal approval. The supervisor owns the fresh Outcome after reading the durable audit. No ready, merge, approve, label, governance or candidate action is authorized.

Outcome: blocked
Reporter: Task #164 supervisor
PR: #169, OPEN/DRAFT, head 461e2fb, base main a897a67, merge-base 446071c. No merge or formal approval exists.
Implementation, publication and the required independent audit are now verified. The earlier blocked Outcome 5875732461 remains accurate historical evidence of resolved publication issues. This Outcome remains blocked ONLY on the existing human PR approval/merge authority and post-merge AC7, not an implementation defect or a new approval stage.
Evidence:
Independent audit: #169 (comment) . Recommended disposition: pass for implementation/evidence audit, no corrective implementation change requested; AC5 and AC7 expressly pending. Reviewer tool-visible session 771177fe-02c5-4b83-9dd4-55bbfecab637, app session 933ed987-ffdb-4049-9ff6-33cc1b0cd7e7, custom reviewer/gpt-6-astra/autopilot, coordination false. Supervisor read the complete audit and verified author mochan-tk/OWNER, unedited publication, matching head and current checks. No second reviewer exists or is needed.
Current verification: all five checks pass at exact head: quality/copilot-surface/scaffold-self-check/windows-launcher from https://github.com/mochan-tk/agentic-dev-kit-for-copilot/actions/runs/36466919136 ; latest task-ritual from https://github.com/mochan-tk/agentic-dev-kit-for-copilot/actions/runs/36468097809 . Worker mandatory local suites report 10 files / 221 cases / zero failures, plus surface/template/direct-consumer/diff/status/count evidence. Supervisor and reviewer inspected records instead of duplicating suites. Compatibility green is not substituted for AC2/AC3. PR stays draft, formal reviews empty, mergedAt null.
Deviations: resolved owner-authorized transport/publication route only: failed HTTPS push preserved historically; exact-ref SSH push and coordinator publication of unchanged worker-authored body under owner identity, with standing publisher resolution 5876376936. Relevant owner records: 5875589353 and 5876152046. No implementation scope change, new approval stage, code/test edits, workflow/check weakening or candidate/history mutation. The worker will prepare an evidence-only linkage update for this audit and Outcome; the coordinator publishes exact inspected bytes per the standing resolution.
Follow-ups: coordinator obtains the owner decision on PR approval and merge. Only after verified merge and explicit instruction may the supervisor perform AC7 candidate fix-link/manual disposition and Task post-merge evidence/Outcome/manual disposition. Preserve existing worker and reviewer sessions for rework/leaf-first teardown; no archive/discard now. No ready/merge/approve/label/governance/candidate action taken by this supervisor.

Outcome: completed
Reporter: Task #164 supervisor
PR: #169, MERGED at 2026-09-29T00:50:01Z; merge commit 70c729c, implementation head 461e2fb. Supervisor independently read mergedAt, merge commit and exactly the two approved files after the coordinator sent the explicit AC7 post-merge instruction on 2026-09-29.
Evidence:
Durable post-merge links:
Deviations: disclosed owner-authorized SSH transport and coordinator transmission of worker-authored PR body per 5875589353/5876152046/5876376936 only; no implementation scope change, new approval stage, rewritten history or weakened check. Earlier blocked Outcomes remain unchanged and accurately describe their earlier states. No edit of the merged PR body is attempted; this record supplies final post-merge status.
Follow-ups: after readback of this Outcome, manually mark Task #164 completed and verify its state, then report the comment ids and dispositions to the coordinator. Retain worker and reviewer without teardown until the coordinator separately verifies AC7 and instructs leaf-first teardown. No branch deletion, labels, governance changes or other-issue actions are authorized or performed.

Releasing worker: Task #164 worker (session 152eb473-d330-4964-a56d-b5c25064e0f7), branch liner-takashi-kawamoto-task-164-retro-completion-evidence
Task: #164
Closeout only: no new Plan, replacement worker, implementation release or scope change. Task #164 supervisor acts on the explicit Epic #119 coordinator teardown instruction after independent AC7 verification and the recorded owner GO #169 (comment) (timing correction 5881498532).
PR #169 merged 2026-09-29T00:50:01Z as 70c729c, retaining implementation head 461e2fb. Candidate fix-link 5881521671 and completed Outcome 5881527645 precede the verified completed dispositions of #125 and #164.
Pre-teardown ground truth: authoritative session status reports both children idle and is_running=false, with no input or Plan gate. Worker 152eb473-d330-4964-a56d-b5c25064e0f7 (app workspace e6a7128f-0bd7-4f1a-9ebf-833121d3eca3) has empty git status, HEAD 461e2fb equal to the retained remote branch. There is no uncommitted work. Implementation is preserved in the retained branch and merged PR; evidence is preserved in the PR, audit and Task records. Reviewer 771177fe-02c5-4b83-9dd4-55bbfecab637 (app workspace 933ed987-ffdb-4049-9ff6-33cc1b0cd7e7) has empty status, zero working diff, HEAD a897a67 and no implementation work to preserve; its durable audit is PR comment 5876549628. Local scratch artifacts are disposable evidence copies, not unrecorded deliverables.
Disposition authority: Epic #119 coordinator under the owner GO and explicit teardown instruction; Task #164 supervisor executes only this closeout. After this comment is read back unedited, archive the worker first and verify archived=true; only then verify/archive the reviewer and confirm archived=true. Preserve both remote branches and all GitHub records. No branch deletion, label changes, existing-comment edits, other-issue action or self-archive. Report both archive confirmations to the coordinator; it alone handles the supervisor teardown afterward.
