The coordinator's leaf-first teardown instruction for Task #138 assumed the mandatory custom reviewer was a child session to archive, as it had been for Tasks #134 and #135. For #138 the reviewer ran as a synchronous read-only task-tool subagent (Task 138 contract audit, verdict 5739774920 on PR #144) with no app session, worktree or branch; the supervisor flagged the mismatch before writing and the coordinator verified independently that only the supervisor and worker sessions existed. Both teardown records had to spell out the N/A. The session-orchestration teardown steps do not distinguish task-tool subagents from child sessions.
