Generated plugin SQL now quotes catalog identifiers and qualifies extension types, functions, and operators by their introspected namespace. Ambiguous or missing service/schema identity fails closed. This covers bulk mutations, i18n, LLM RAG, ltree, PostGIS, and search.

Rebased onto current main; the performance harness prerequisite has landed. The change remains independently reviewable from cache ownership (#1751).

Validation: frozen install, affected package CJS/ESM builds, and 11 focused suites passed. Full CI passed all 17 jobs on 6dc1bbc25bd2cc98aa476783a2917559177f22fe.
