stream_body/1is gone on 3.x andwith_bodyis ignored, so the README examples no longer ran. Cleaned up Quick Start and Streaming, droppedwith_body, and replaced "server push" with "flow control" in the HTTP/2 guide blurb. Remaining snippets verified against httpbin and nghttp2.
