Broken README links (all verified 404 → 200)

Remove docs/IMPLEMENTATION_PLAN.md

A v0.1-era roadmap, fully superseded: all four phases landed, the io
vtable changed (submit/wait → read/write/flush/close + readv/writev),
macOS support shipped, and the file list is stale. SESSION_STATUS.md
is the living status doc. The README project-structure section is
updated; the only remaining mention is the historical Phase 0 record
in SESSION_STATUS.md.

Verification

All 7 README links (5 http + 2 relative) ✅ HTTP 200 / files exist
docs/IMPLEMENTATION_PLAN.md ✅ deleted
Dangling references in README/docs ✅ none

- Build badge pointed at a non-existent .../badge.svg URL; use the
  GitHub Actions workflow badge (workflows/CI/badge.svg)
- Releases link used 'Avers/flashcheck' (wrong repo name)
- Actions link used 'flash-checker/flash-checker' (wrong owner)
- delete docs/IMPLEMENTATION_PLAN.md: a v0.1-era roadmap, fully
  superseded (all phases landed, interfaces changed); SESSION_STATUS.md
  is the living status doc. README project structure updated.

All README links verified (HTTP 200 / files exist).
