Support for chapter subfolders in the table of contents for comic books

Thanks for the great app! Special thanks for the comic book support! Apart from Sumatra, I haven't seen any other readers that support webcomics (long scrolls).

Please add support for chapter subfolders in the table of contents for comic books. I created an example in GIMP.

I've changed the image a bit—added page numbers for the chapters. If it's not too much trouble.

kjk, thanks for adding this feature! I'm waiting for the release version.

PS. In the advanced option is missing PageSpacing for ComicBookUI, although it's still in SumatraPDF-settings.txt. Should I create an issue?
